PHYSICAL EXAM and HEALTH HISTORY

 
 

High school students who are involved in school sport activities are required to have a physical exam by a physican prior to any sports event or try outs.

New to district students transferring into the Anchorage School district from another school district and students who are in grades K, 1, 3, 5, 7, and 10 will receive a health screening by the school nurse. The school nurse will review the student's health history form, screen for vision, hearing, height, weight, color test and tuberculosis - if needed. Tuberculosis screening is done with parent consent. Parents will be notified of any health problems identified during the heath screening. Kindergarten students are required to have a school physical within 90 days of enrollment or provide the school with a copy of an exam completed within the last year.

Each year parents are required to register their student for school and complete a current health history form. The school nurse reviews the health history form. Parents are contacted when the school nurse has any questions or concerns. Parents are encouraged to call or visit their school nurse and conference regarding the special needs of their child. This is especially critical for students with special medical conditions or when there has been a significant change in a child's health.
